Eptifibatide is a cyclic heptapeptide derived from a disintegrin protein (P22827) found in the venom of the southeastern pygmy rattlesnake (Sistrurus miliarius barbouri). It belongs to the class of the arginin-glycin-aspartat-mimetics and reversibly binds to platelets. Eptifibatide has a short half-life. The product is the third inhibitor of GPIIb/IIIa that has found broad acceptance after the specific antibody abciximab and the non-peptide tirofiban entered the global market.

Eptifibatide peptide has a modified amino acid chain of lysine glycine aspartic acid, which can bind to GP IIb/IIIa receptors and block GP IIb/IIIa receptors, thereby preventing coagulation factor I from binding to platelet GP IIb/IIIa receptors and exerting antiplatelet aggregation effects. The inhibitory effect of Eptifibatide peptide on GP IIb/IIIa receptors is competitive inhibition, and the inhibitory effect is concentration dependent. In addition, the binding of eptifibatide to GP IIb/IIIa receptors is reversible, and the effect is reduced by half 4 hours after discontinuation. The drug efficacy can be rapidly reversed with the termination of administration, and treatment does not increase the risk of bleeding. And studies have shown that administration of eptipatide did not detect serum antibodies or immune reactions. |

Additionally, hypotension was seen frequently (6%). Cardiovascular failure was also frequent (2%) as were serious arrhythmias (ventricular fibrillation 1.5%, atrial fibrillation 6%). Severe allergic (anaphylactic) reactions occurred in almost 0.2% of patients.
