Description

Sulbactam powder is a white crystalline solid at room temperature and pressure, with a melting point of 148-155 ℃. It has good water solubility and is easily soluble in common alcohol organic solvents and ethyl acetate. Pure sulbactam acid is a semi synthetic β - lactase inhibitor, therefore it has a strong irreversible inhibitory effect on penicillinase and cephalosporinase. But its antibacterial activity is weak, so it is rarely used alone. In clinical practice, this product is often used in combination with β - lactam antibiotics, which has a significant synergistic effect and good antibacterial effect, and is effective against most drug-resistant bacteria.

What is the difference between β -lactam antibiotics and β -lactamase inhibitors?

β - lactam antibiotics are a widely used class of antibiotics in clinical practice, but some pathogenic bacteria can produce beta-lactamase, which hydrolyze the β - lactam ring of antibiotics and lose drug activity, thus leading to the occurrence of drug resistance.

The solution is to add beta lactam enzyme inhibitors (such as clavulanic acid, sulbactam acid, and tazobactam) on the basis of beta lactam antibiotics, because these three β lactam enzyme inhibitors all have a beta lactam ring, which can competitively bind with β-lactamase to prevent the hydrolysis process of the beta lactam ring.

The combination of β - lactam antibiotics/beta-lactamase inhibitor can significantly reduce the minimum inhibitory concentration of methicillin-resistant Staphylococcus aureus, Haemophilus influenzae, Escherichia coli, Klebsiella pneumoniae, Bacteroides fragilis, and Proteus mirabilis.

Clavulanic acid is a typical representative of oxapenams inhibitors (or Clavams inhibitors ), which was first isolated from Streptomyces fermentation broth in 1976. Scholars at home and abroad also synthesized penicillanine sulfones from 6- aminopenicillanic acid (6-APA), such as sulbactam and tazobactam.(Tazobactam adds a triazole ring to the structure of sulbactam.)

The enzymatic intermediates of these three β - lactam compounds can form a strong irreversible inactivation enzyme complex with β - lactam enzymes, enhancing the antibacterial activity of β - lactam antibiotics and significantly reducing their MIC and MBC. Therefore, they are sometimes referred to as "suicide enzyme inhibitors".

Difference in Enzyme Inhibition Activity

1. Clavulanic acid exhibits strong inhibitory activity against plasmid mediated β -lactamase, but is ineffective against chromosome mediated β -lactamase.

Sulbactam is stable and has a long half-life. In addition to the plasmid β -lactamase, it also has a certain inhibitory effect on the induction enzymes produced by chromosomes. The inhibition spectrum is relatively wide and not completely the same as that of Clavulanic acid sensitive bacteria.

A study has found that the inhibitory constant of Sulbactam on enzymes is greater than the CLA value, and the enzyme inhibition activity is higher than that of CLAV.

2. Tazobactam has a wider enzyme inhibition spectrum, stronger tissue penetration, and wider distribution. It has inhibitory effects on chromosomes, plasmid mediated and various broad-spectrum enzymes, penicillinase.

Studies have shown that compared to Clavulanic Acid, Tazobactam has a significantly increased inhibitory effect on cephalosporin. Meanwhile, some scholars have found that Tazobactam has a higher inhibitory constant value on enzymes than Sulbactam.

There are also literature reports that Tazobactam is 10-20 times more sensitive to TEM enzymes producing Escherichia coli than clavulanate and Sulbactam, but Tazobactam and Sulbactam have roughly the same efficacy against Pseudomonas aeruginosa.

To sum up, the inhibitory activity is tazobactam > sulbactam > clavulanic acid.

Difference in enzyme production ability of induced bacteria

The induction of bacterial enzyme production by Tazobactam is significantly lower than other beta lactase inhibitors (Clavulanic Acid and Sulbactam), and its ability to induce bacterial resistance is extremely low, with a high degree of production The frequency of mutant strains of β - lactamase is also low.

Differences in clinical applications

Clavulanic acid is the earliest beta-lactamase inhibitor used in clinical practice. Currently, CLAV can be combined with two antibiotics, Amoxicillin and Ticarcillin.

Sulbactam pivoxil was first synthesized in 1978 and is the enzyme inhibitor with the largest variety of combination drugs in China.
At present, Sulbactam can be combined with seven antibiotics including Piperacillin, Cefoperazone, Mezlocillin, Ceftriaxone, Cefotaxime, Ampicillin, and Amoxicillin.

Tazobactam was synthesized in 1980, and piperacillin/tazobactam was the earliest combination preparation on the market.

At present, tazobactam is combined with piperacillin, cefoperazone, ceftazidime and ceftriaxone.
