Mexiletine hydrochloride powder CAS 5370-01-4 is a sodium channel blocker-type antiarrhythmic product. Its chemical structure and electrophysiological effects are similar to lidocaine. It acts as a membrane stabilizer, blocking Na+ channels in the cell membrane and inhibiting Na+ influx, thus shortening the action potential, relatively prolonging the effective refractory period, and reducing excitability. Therapeutic doses have minimal effect on conduction in the sinoatrial node, atria, and atrioventricular node. In individuals with normal conduction systems, it has no significant effect on the automaticity of the sinoatrial node, atrioventricular conduction, QRS waves, or Q-T interval. Therefore, its electrophysiological effects vary depending on the dose and myocardial state (e.g., normal or ischemic, hypoxic). High blood concentrations can significantly prolong the refractory period of myocardial conduction fibers. This product has almost no inhibitory effect on the myocardium.

Mexiletine is an antiarrhythmic product (Class Ib) with a structure similar to lidocaine. Mexiletine inhibits the influx of sodium ions required for impulse initiation and conduction, thereby reducing the rate of action potential rise. It achieves this reduction by inhibiting sodium channels. Mexiletine reduces the effective refractory period (ERP) of Purkinje fibers in the heart. The reduction in ERP is less than the reduction in action potential duration (APD), leading to an increase in the ERP/APD ratio. It does not significantly affect resting membrane potential or sinoatrial node automaticity, left ventricular function, systolic arterial pressure, atrioventricular (AV) conduction velocity, QRS, or QT interval.

Mexiletine has a rapid onset of action and counteracting kinetics, meaning it has little or no effect at slower heart rates and a greater effect at faster heart rates. It shortens action potential duration, reduces refractoryness, and reduces Vmax in partially depolarized cells with rapid-response action potentials. Mexiletine does not alter or reduce the duration of action potentials. Mexiletine is well absorbed from the gastrointestinal tract (bioavailability is 90%).
