Tertomotide works via two separate pathways:
1. Therapeutic vaccine mechanism: It is taken up by antigen-presenting cells and presented on both MHC‑I and MHC‑II, activating hTERT-specific CD4⁺ helper T cells and CD8⁺ cytotoxic T lymphocytes to kill telomerase-positive tumour cells. It binds a broad range of HLA subtypes.
2. Direct neuroprotective & anti-inflammatory mechanism: As a cell-penetrating peptide, it crosses cell membranes and the blood-brain barrier. It binds bradykinin receptor B1R to trigger the mTORC2‑AKT axis, reprogramming microglia to clear amyloid-beta plaques. It also interacts with HSP70/HSP90 to suppress NF‑κB signalling, stabilise mitochondria, reduce oxidative stress and neuronal apoptosis.
