NO activates guanosine monophosphate cyclase, which converts guanosine triphosphate to cyclic guanosine monophosphate (cGMP), thereby activating cyclic guanosine-dependent protein kinase. This leads to a decrease in calcium ion concentration within smooth muscle cells, causing the smooth muscle of the penile corpus cavernosum to relax and resulting in an erection. Type 5 phosphodiesterase (PDE5) degrades cGMP into a physiologically inactive product, causing the penis to enter a weak state. Tadalafil inhibits the degradation of PDE5, leading to the accumulation of cGMP, relaxation of the smooth muscle of the penile corpus cavernosum, and resulting in penile erection.
