Basic information of omeprazole

Omeprazole powder is white or white-like crystalline powder; Odorless; easy to change color when exposed to light. Soluble in dichloromethane, slightly soluble in methanol or ethanol, slightly soluble in acetone and insoluble in water; Dissolve in 0.lmol/L sodium hydroxide solution.
Omeprazole is a brand-new anti-peptic ulcer drug whose mechanism of action is different from that of H2 receptor antagonism. It specifically acts on Parietal cells of gastric mucosa, reducing the activity of "Proton pump" in parietal cells, so this kind of drugs is also called "proton pump inhibitor". On June 20, 2003, the US Food and Drug Administration approved omeprazole as an over-the-counter drug in the treatment of frequent heartburn.

What is Omeprazole?

Omeprazole (H 16868) is a proton pump inhibitor (PPI), which has the potential to be used in the research of gastrointestinal diseases. Omeprazole competitively inhibited the activity of CYP2C19, with a Ki of 2 to 6 μM m. 

Omeprazole powder also inhibits the growth of gram-positive bacteria and gram-negative bacteria. It is an inhibitor of N-SMase (exosome inhibitor).

 

 
What are the synthetic routes of omeprazole?
 

3,5- dimethyl -2- hydroxymethyl -4- methoxypyridine was chlorinated to produce 2- chloromethyl -3,5- dimethyl -4- methoxypyridine. 4- methoxy 1,2- phenylenediamine reacts with potassium xanthate to generate 2- mercapto -5- methoxybenzimidazole, and then reacts with the pyridine derivative obtained above to generate 2-[(3,5- dimethyl -4- methoxy -2- pyridyl) methylthio ]-5- methoxy -1H- benzimidazole. The crude omeprazole can be recrystallized from acetonitrile.

What effect does omeprazole have?
1. Omeprazole powder is mainly used for duodenal ulcer and Zhuoai syndrome, and can also be used for gastric ulcer and reflux esophagitis.
2. It is a proton pump inhibitor and a fat-soluble weakly alkaline drug. It is easy to concentrate in acidic environment and transform into active sulfenamide. Through the irreversible combination of disulfide bond and sulfhydryl group of proton pump, the activity of H+ and K+-ATPase is inhibited, and the acid content in gastric juice is greatly reduced.
3. Severe renal insufficiency and infants are prohibited. It is reported that atrophic gastritis has been observed in the stomach biopsy specimens of patients who have used this product for a long time abroad. Long-term use may lead to hypergastrinemia and vitamin B12 deficiency.
 

How does omeprazole work?

Like other proton pump inhibitors, omeprazole can bind to H+/K+-ATPase of gastric parietal cells, inhibit its activity and prevent proton pump from transporting H+ to gastric cavity (the last step of gastric acid production), thus inhibiting gastric acid production. Omeprazole, as a prodrug, is converted into active substance (hyposulfonic acid) through acid secretory tubules of gastric parietal cells. Because the inhibition process is irreversible, gastric acid secretion will be limited in the next 24 to 48 hours until new proton pump molecules are synthesized and transferred to the cell membrane.
 
why reduce the secretion of gastric acid?

Omeprazole powder in the treatment of gastric ulcer is mainly related to its mechanism of inhibiting proton pump and reducing gastric acid secretion. Then why reduce the secretion of gastric acid?

This is because after the formation of gastric ulcer, high concentration of gastric acid stimulates and corrodes the gastric mucosa, which causes continuous damage to the ulcer area and hinders the healing process. This includes three common mechanisms:
