Its main mechanism of action is to stimulate insulin secretion from pancreatic β-cells and partially increase the sensitivity of peripheral tissues to insulin. Glimepiride binds to and dissociates from insulin receptors faster than glibenclamide, thus causing less severe hypoglycemia.

Its mechanism of action involves binding to sulfonylurea receptors (proteins with a relative molecular mass of 6.5 × 10⁴) on the surface of pancreatic β-cells. These receptors are coupled to ATP-sensitive K⁺ (KATP) channels, causing KATP channel closure, leading to cell membrane depolarization, opening voltage-dependent calcium channels, Ga²⁺ influx, and thus insulin release, while inhibiting hepatic glucose synthesis. Glimepiride is suitable for type 2 diabetes where blood glucose control is unsatisfactory despite diet, exercise, and weight loss.
